Requesting Your First Appointment

 

Please fill out the above application form if you wish to start working with us. For insurance patients, please send us the front and back of your insurance card and your birth date when you contact us. It usually takes up to a week to verify your coverage. We will get back to you as soon as we can.

Once the appointment is made, we will send you our practice’s policy via a secure client portal. You can review and complete the paperwork, and bring any questions to your first meeting. At this time, we are mostly seeing patients remotely, if you would like to request for an in-person appointment, please let us know.

 

Your First Meeting

 

During your first meeting, we will develop a rapport, get to know each other, and learn how we can best work together. We will gain an understanding of your presenting concerns, relevant history, and your goals. We will also assess your strengths as well as areas of growth.

 

Fees/Insurance

 

At Mind in Motion, we strive to provide comprehensive and hassle-free billing services, whether we are in-network or out-of-network with your insurance. Rest assured that we will verify and handle all of your billing needs, ensuring a smooth process for you. ​

We are in network with Aetna, Optum (UHC, Oxford, UMR, etc), Cigna and Medicare. ​

While we take care of the verifications and claims process, it is important for you to have a clear understanding of your insurance coverage. We encourage you to get in touch with your insurance provider and ask the following questions:

– How much is my deductible, and has it been met?
– What is my copay and/or coinsurance?
– When is my policy renewal? Remember, this resets your deductible.

If you are considering paying out of pocket, we are more than happy to provide you with our fee schedule upon request. Additionally, we offer sliding scales based on your income and availability.​

We accept cash, checks, debit/credit card and HSA account.​

Cancellation Policy
Our practice maintains a cancellation policy of 1 business day or 24 business hours before your appointment time. There is a full session fee applied for all cancellations under 24 hours. For insurance clients, please be aware that the insurance rate will be charged to you.

Late Show
If you are late more than 15 minutes without advanced notice, we would consider the session as a no-show. Even if the session started late, we will still have to end the session at the allotted time.
